What is Melanoma Treatment and Reconstruction?

Melanoma is a type of skin cancer that develops from melanocytes, the cells responsible for producing melanin, the pigment that gives skin its color. It usually occurs due to prolonged exposure to ultraviolet (UV) radiation from the sun. Melanoma can develop anywhere on the body but is most commonly found on the chest and back in men and the legs in women. It can also appear on areas not commonly exposed to the sun, such as the soles of the feet, palms, and under fingernails.

Melanoma treatment and reconstruction involves a combination of medical and surgical approaches to remove cancerous tissues and restore the appearance and function of the affected areas. Treatment and reconstruction often involve a team of specialists, including dermatologists, surgical oncologists, plastic surgeons, medical oncologists, radiation oncologists, and other healthcare professionals to provide comprehensive care.

What Does Melanoma Treatment and Reconstruction Involve?

Surgical Treatment:

  • Wide Local Excision: The primary treatment for melanoma is surgical removal with a margin of normal skin to ensure all cancer cells are removed.
  • Sentinel Lymph Node Biopsy: To check if the cancer has spread to the lymph nodes, a sentinel lymph node biopsy may be performed during surgery.

Advanced Surgical Techniques:

  • Lymphadenectomy: Removal of nearby lymph nodes if cancer has spread.
  • Mohs Surgery: A precise surgical technique where thin layers of cancer-containing skin are progressively removed and examined until only cancer-free tissue remains. This is particularly useful for melanoma in areas where cosmetic and functional preservation is important.

Non-Surgical Treatments:

  • Immunotherapy: Uses medications to stimulate the immune system to attack melanoma cells. Drugs include checkpoint inhibitors (e.g., nivolumab, pembrolizumab) and cytokines (e.g., interferon-alpha).
  • Targeted Therapy: Involves drugs that target specific genetic changes in melanoma cells. Common targets include BRAF mutations (treated with BRAF inhibitors like vemurafenib and dabrafenib) and MEK mutations (treated with MEK inhibitors like trametinib).
  • Radiation Therapy: Used to treat melanoma that has spread to other parts of the body or to relieve symptoms.

Adjuvant Therapy:

Treatment given after primary surgery to reduce the risk of melanoma returning is called adjuvant therapy. This can include immunotherapy, targeted therapy, or radiation.

Reconstruction After Treatment

  • Skin Grafts: This involves transferring healthy skin from another part of the body to cover the surgical area.
    • Split-thickness Skin Grafts: Uses the outer layers of skin from a donor site.
    • Full-thickness Skin Grafts: Includes all layers of skin and are used for more extensive reconstruction.
  • Flap Surgery: Involves moving tissue from one part of the body to another, maintaining its blood supply. This can be crucial for large or deep excisions, particularly on the face.
  • Microsurgery: Advanced techniques involving a microscope to reconnect tiny blood vessels and tissues, allowing for precise and effective reconstruction.
  • Scar Revision: Procedures to improve the appearance of scars left after melanoma excision. Techniques include laser therapy, dermabrasion, or minor surgical corrections.
  • Prosthetics and 3D Printing: Custom prosthetics, especially for facial reconstruction, using advanced 3D printing technology to create parts that blend seamlessly with natural skin.

Follow-Up Care

  • Regular Skin Exams: Continuous monitoring for new melanomas or recurrence.
  • Imaging Studies: Periodic scans to check for metastasis in high-risk cases.
  • Sun Protection: Consistent use of sunscreen, protective clothing, and avoiding peak sun hours to prevent further skin damage.
  • Support Services: Access to counseling and support groups to help cope with the emotional and psychological impact of melanoma.
